Terms

1. I/my child shall adhere to suggested safety precautions and protocols at the ground.
2. I hereby acknowledge that there might be inherent risks associated with participating in football that may result in personal injury
3. The Player (or if the Player is under 18 years of age at the time of submitting this form, the Player's parent or legal guardian) agrees to fully bear the risk of Covid-19 and personal injury
4. I release Game On Sports Management Pvt. Ltd, its staff, partners, sponsors and agents, from all claims arising from any injury/illness that I/my child may suffer because of participating in the program. In doing so, I acknowledge that I am discharging Roots Football School, Game On Sports Management Pvt. Ltd., its staff, partners and agents, from all legal responsibility for any injuries/illness suffered by my child from participating in the program.
5. Roots Football School, Game On Sports Management Pvt. Ltd. may use the Player's name or image in any form or medium for general marketing and promotional activities
